I got the software recall fix last week. Today is the first time I have had any issues. 2 time today the emergency braking activated. First time I was leaving McDonald’s parking lot. I waited for a car to pass then I took off to get on the road. The emergency braking system activated leaving me halfway in the parking lot and part way in the road. I thought that was weird. The a few minutes later I arrived at home and started backing into the driveway like I always do. As I was backing into the driveway a car started to pass on the road in front of me. The car emergency braking system activated leaving me part way in the road and driveway was the car passed. I have saved the ring camera footage of me arriving home in case it is needed.

The contact owns a 2022 Toyota Sienna. The contact stated while driving approximately 40 MPH and slowing to stop at a traffic sign, the brake pedal was depressed and there was no resistance from the brake pedal. The contact depressed the brake pedal with force to bring the vehicle to a stop. The contact stated no warning light was illuminated. The contact had driven the vehicle to a local dealer, where it was diagnosed and found that the brakes were operating correctly. The vehicle was not repaired. The contact stated that the failure recurred intermittently. The manufacturer had not been informed of the failure. The failure mileage was approximately 10,000.

This van was converted to be wheelchair accesible by Vantage Mobility International. My parents have repeatedly had an error arise that indicates an issue with the hybrid battery cooling system. This system was modified by VMI, in partnership with Toyota, to allow for the accessibility features to be added to van. Neither the vehicle dealer, Superior Van and Mobility in Knoxville, TN, nor the local Totota dealer, Toyota of Knoxville, have been able to rectify the issue, despite mutliple attempts.We are concerned that this is a safety issue, and have asked for written confirmation of that and for the issue to be resolved. None of the three parties, Superior, Toyota, nor VMI, have been willing to do that nor rectify the issue. (I have filled this out for my parents because this has already become so stressful and in the wake of my dad's rapidly progressing disability, I don't want either of them to have to deal with more of the headache with this.)

The TPMS sensors stop working when driving at highway speeds and won't tell you the air pressure. Says see dealer but when I take it there, they can't do anything. Has happened probably 10 times. Usually only on interstate. Can be safety hazard if tire pressure goes low and no idea.

The 3rd row seats doesn’t have locks when it folds down so when we hit a bump it just go up and down and during sudden breaking the folded 3rd row seats come up because there is no locks . But all older models of Sienna have the safety lock . It is a simple fix but my new Sienna doesn’t have the major safety lock . During a accident it might come and hit the 2nd row passengers .PLEASE CONSIDER THIS SAFETY ISSUE AS SOON AS POSSIBLE.

3rd row seats do not lock down when stowed. When vehicle comes to abrupt stop, 3rd row seats shoot up with high velocity. Anyone in the vicinity will be harmed. Any items placed on chairs will fly as projectiles, harming individuals with the possibility of an accident.

We have an rear entertainment center package in our 2022 Toyota Sienna van, when we pull the screen down it completely blocks our rear view due to screen is too big. I am wondering why Toyota didn’t install digital rear view mirror to all trim levels with rear entertainment center, but they automatically install the to limited and platinum trims. To us that is an safety issue, for instance if there is a cop, fire truck or ambulance coming up behind us we wouldn’t see them do to it blocking our view. I have a 12, 8 and 3 year old and everytime they get in van they want to always watch the rear entertainment center. Thanks.

Got my 2022 sienna with the factory installed rear entertainment system. When the entertainment system is in use, there is ZERO visibility out of the rear view mirror or window. This is a huge safety issue as I have almost been rear ended 4 times now. Today while it was snowing, if I had my entertainment system in use, I would not have been able to see that someone was sliding in to me. Luckily I saw out of my rear view mirror and I was able to hurry and speed up to not get hit. I have taken the car to 2 Toyota dealerships who have both verified and took pictures to show that you cannot see a thing out of the mirror or window when the entertainment is in use. They all agree it is a major safety issue and reported it to Toyota corporate and opened a case to get it fixed. The fix is to install a digital rear view mirror for those who have the entertainment system (which again comes from the factory) Toyota brand engagement said that yes this is a safety issue but since there is no ope recall they won't fix it for me by giving me a digital rear view mirror. This is absolutely crazy that there is this big of a design/safety flaw over looked by Toyota. They also told me that there have been some that have been fixed however it is a case by case basis and basically depends on who you talk to. I just paid 52,000 for this car and I can't use the features it comes with because of this huge safety issue. If I had my back window taped up completely blacked out and couldn't see through it, I would get a ticket from the police for a safety hazard of not being able to see out of my back window. How is this different? This is unacceptable and should immediately be recalled for those vehicles that toyota put entertainment systems in. This issue could cause people to be rear ended because they cannot see anything out of their windows. I can send pictures that the dealership took to verify this safety issue. It needs to be addressed and corrected before people get hurt.

The car does not have DIGITAL REAR VIEW which uses built-in camera installed at bottom of back to display what is behind the car. Because of this whenever ENTERTAINMENT SYSTEM DISPLAY (DVD PLAYER) is pulled for use or CARGO is fully loaded; REAR VIEW MIRROR does not show anything and driver remains unaware what is coming behind the car or what is going on behind the car. I had incident in first couple of usage it self that I was not able to see emergency vehicle & was not able to give a way. Additionally, it could be difficult to understand behaviour of other cars behind coming. I have purchased this car paying lots of money and others could have do so thinking lots of safety features for family usage purpose. Unavailability of rear-view mirror display when there is obstructing entertainment system and cargo, will completly put all passengers at risk. I have paid more than MSRP for this car as I was in deep need of car. Dealer did not give any detail on this issue and when drove car multiple time then only experienced. Raised everything to TOYOTA by calling their number given in the car manual. No one picks up. Submitted online complaint (Reference#220311002236) but not getting any helpful response. They are just saying someone will call from nearest toyota dealership or reachout directly. When no call came, went to dealer who says reachout to TOYOTA directly! Its going in circle. Also reached out to DEALER from where it is purchased, they are too not responding. This is to be taken seriously. I am going thourgh lots of harrassment by dealer/toyota after paying this much for the car and should be compensated accordingly. NHTSA should help me getting DIGITAL REAR VIEW CAMERA installed in this vehicle and have TOYOTA to install same in all other similar configured TOYOTA SIENA vehicle. As an alert citizen and driver, I am raising this and can provide more details if needed but need all help to fix on this car. Rest everyting is fine in the car. Thanks.

When using polarized sunglasses, visibility of the driver and passenger front windows gets obstructed (standard windows, no modification) . This means, that these windows are partly intransperant. In an irregular pattern clouds seem to be dispersed all over them. In total maybe one third or up to 50 percent of the window seems to be way less transparent, only while wearing polarized glasses. Without glasses there is no impaired visibility at all.

The contact stated that there was no warning light illuminated showing that the accelerator and brake pedals were being depressed simultaneously. The contact was able to drive home safely. The vehicle was not yet taken to be diagnosed. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 1,600.

When breaking at a stop sign or light, the vehicle occasionally feels like it surges forward and I'm accelerating instead of breaking. This causes me to push harder on the brakes, which then seem to mechanically engage and then stop the van. I've had the van for a year now and it happens a few times a month, especially if I breaking on an uphill incline.

BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING

While operating the vehicle in the city of Helena, Montana, foreign matter entered the interior of the rear bumper. This caused the RCTA and BSW systems to fail because the sensors were obstructed by mud. This failure condition happens to my 2022 Toyota Sienna every three weeks. Toyota has designed the vehicle to allow the ingress of debris to the *interior* of the rear bumper, therefore the failure of RCTA and BSW is inevitable. Toyota can and should know that this failure is inevitable but has offered no repair, nor has it offered advice on how to restore operability after failure. Yes, mud can interfere with safety sensors on any vehicle, but only this 2022 Toyota Sienna intentionally allows mud to accumulate on the *interior* of the bumper resulting in this otherwise avoidable failure.

We are not talking about malfunction, since it's underperforming by design. Rcta and BSM don't make any audible alert. Every other vehicle I owned had this. Here BSM is silent and rcta can't be heard, because volume can't be adjusted. This is dangerous, since relying on visual with rcta doesn't make sense, when you try to watch you back in reality through the rear windows. Please recall this toyota safety issue because it doesn't utilize its potential and poses an unnecessary risk for everyone.

During heavy rain exhaust system become dislodged and fell to the road, only hanging on by the engine connection. We had the vehicle towed to the dealership only to be told that is not a warranty issue. They did reinstall it in the rubber brackets. Due to my persistence a warranty claim was opened. There claim was that it hit something as there were marks across the muffler however those were made by dragging on the road until vehicle could be driven off the road. Knowing it was most likely water pressure from the water on the road that lifted the exhaust up and out of the rubber brackets, I wanted this documented. Although if the exhaust system had completely broken off from the engine it would have not hurt our vehicle occupants, any vehicles behind could have been hit.

I my Sienna had its 12 volt battery depleted over night with no reason. After it was jump started, literally every single piece of equipment was malfunctioning and producing an alert (hybrid system, brake pressure, RCTA, BSM, AWD,tire pressure and so on). After several restarts of the engine (about 15 min after running engine), all systems went to normal except hybrid drive system, that still asked for maintenance and the tire pressure sensor was still on alert. However I drove the vehicle to a dealership in Maine after the parking brake finally decided to get loose again. These guys told me, they had just the same problem with another 2022 sienna a week before. However, they reseted the errors, and everything worked fine. They had no explanation why two sienna in one week come to their lot with a depleted battery and as a consequence with a malfunctioning hybrid systen. That's very unsatisfying.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

This Hybrid LE model brake system is very poor. If we press the break pedal it stops 5-10 meter far. Urgently need for recall and rectify this major issue to avoid more accidents and property damage to others on the road .

SRS Airbag warning light is in the dashboard. It is confirmed by the Toyota dealer a faulty radio head unit can not register the SRS airbag. In the event of collision, the SRS airbag will not be deployed, causing passengers at risk.

After starting the van, an error pops up: "SRS Airbag System Malfunction Visit Your Dealer." This occurs a day after turning the car on and off and driving for several hours. The next day, it may come back on, or it may not. I have taken it to a dealership, and they stated it was a firmware update they completed. After they updated the warning light, it came back on less than a week later.

Over the past 18 months, this vehicle has exhibited the following actions approximately 30-40 times. When slowing the vehicle, with brake pressure applied, the vehicle reacts appropriately until, at the slow speed( approximately 5-10 mph) it suddenly accelerates or surges ahead for approximately 1-2 seconds. The has occurred while in the process of turning as well as moving straight ahead. I have taken the vehicle to the dealer of the original sale, Shepard's Toyota, Thomaston Maine, and they have attempted to replicate this action. They were unable to do so. The reported to me that they contacted Toyota engineering and was told, I paraphrase, that when the drive battery is fully charged and the dynamic braking system is still creating a charge, the current is directed to the drive motors, despite the fact that the brake is applied to slow and stop the vehicle. These events have occurred inn the middle of turns, when attempting to stop at crosswalks, and stopping prior to entering a traffic stream. If I had not continued to apply the brake, the vehicle would have, in several case, collided with objects.

Occasionally there is a message popping up about a malfunction of the tire pressure sensor while driving. This appears in conjunction with the yellow flat tire warning. It is very inconvenient, since I have to stop the van just to find out that the tire pressure is fine. Actually it's more than inconvenient since this may lead to a kind of negligence of this system what poses a safety thread. On top of that, the Toyota connect is app keeps telling me, that my tire pressure is low. Went to the dealership, they say they can reproduce the error message and the problem is known to Toyota. However they can not fix it. It seems to be a software issue.

A tire and wheel came off of my 2022 Toyota Sienna while driving over a mountain pass. I was going downhill and around a curve, heard and felt some clunking, then watched my tire bounce across the two lane highway and over the steep dropoff. This was a potentially fatal occurrence, for so many reasons. This malfunction put the oncoming traffic and the drivers behind me at risk for collision, or worse. There was no warning lamp on, my tire pressure sensors read steady, and no other system lit up before or during this occurrence. The dealer can not find a reason as to why this happened within a year of owning it.

EXTERIOR LIGHTING

Question 1 Auto Headlight continues to malfunction and it has been inspected 3 times by Randy Wise Toyota (Grand Blanc, MI). Yes, it is available for inspection (video & list of incidents provided to Randy Wise Toyota - as requested by Tech/[XXX] but no action taken). Question 2 High Beams have come on when oncoming vehicle is between 300 to 500 feet - potentially temporarily blinding the person in the oncoming car and causing an accident. The High Beams also have come on when following vehicles within 300 to 500 feet, also in the City (with city lighting) the high beams were on with a City Bus approaching 1 block away. We have had numerous cars flashing their bright lights at us as our high beams do not dim to low beams appropriately potentially temporarily blinding us and creating an accident. We also are concerned about a potential road rage incident due to others thinking we are intentionally turning on our high beams. Question 3 No the dealership Service Director [XXX] took our vehicle home (23 miles each direction - during the day time) but only test drove the vehicle for 6 miles to try to duplicate the issue. When asked how many vehicles he passed, was told he was not available (out of town) we asked for a phone call to discuss this - but never received a call. Tech/[XXX] stated that we needed to provide video documentation and to keep a list of when the issue happens and details - which was provided but since there was no "CODE" they determined there was no problem. Question 4 Vehicle has been inspected by Randy Wise Toyota 3 times. Question 5 No warnings prior to failure. First appears immediately after purchase. Add'l Info: Video provided to dealership shows us following a vehicle (never loosing site of their tail lights) when an oncoming vehicle approached and when the oncoming vehicle was approx. 500 Ft. in front of us our bright lights turned on and remained on until that oncoming vehicle was approx. 300 Ft. from us. FORWARD COLLISION AVOIDANCE: WARNINGS

When pulling out of a parking lot, the accelerator disables leaving the car unable to move in the middle of the rode for a few seconds. The happens intermittently and usually when there is traffic going both ways in the area. This has happened several times but has not been replicated with a dealership employee during test drive. After the accelerator disables, it will enable after a few seconds. It has left me sitting in the road with cars honking at me. Van is currently in the shop. Brake pedal was not deployed at the same time of acceleration.

How was your safety or the safety of others put at risk? None Stop beeping Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, was replaced once under of warranty and failed again. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No, failed again after 30,000 miles

VISIBILITY:SUN/MOON ROOF ASSEMBLY

The contact owns a 2022 Toyota Sienna. The contact stated that while driving approximately 35 MPH, the sunroof fractured without impact. The contact called the local dealer and was informed that the sunroof glass needed to be replaced. The vehicle was being repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 10,000.

The contact owns a 2022 Toyota Sienna. The contact stated that while his wife was driving at 65 MPH, a loud sound was heard. Upon inspection, the contact noticed that the sunroof had exploded; however, the sunroof panel was closed, and no glass entered the interior of the vehicle. There was no impact on the sunroof that could have caused the explosion. The vehicle was taken to the dealer, and the dealer referred the contact to take the vehicle to Vantage Mobility International (VMI, 5202 S 28th Pl, Phoenix, AZ 85040) due to the wheelchair van conversion. The vehicle was taken to VMI, where it was determined that a small impact caused the sunroof to explode, and the sunroof needed to be replaced. The vehicle was not yet repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 750.

POWER TRAIN

When decelerating, particularly on hills or into corners, there is an intermittent surge that occurs after the brake pedal has been depressed. It feels as though there is a release of the braking action, this is unsettling as if you are starting to approach a vehicle or pedestrian, this sudden "delay" of braking could result in contact with other objects. We have changed our driving behavior as a result of this. We reported this behavior at 5000 miles and there was not a solution until last year. We were not notified of this, and our vehicle is now out of warranty. Toyota has released as of last year a TSB related to programming and we are going to pay out of pocket $300 this week to get that TSB applied. This doesn't seem fair to consumers. You can see this is not an isolated problem here: [XXX] , a 2025 and 2024. In both of those vehicles, this behavior is not present. The Entune 3.0 System governs navigation, calls etc. The system on my car has been replaced 4 times and still does not work. This is a life safety issue as the system grabs phone calls while driving without being able to answer. It also makes it frequently impossible to use hands free on the phone as it grabs calls. In order to avoid it grabbing calls I have to disable bluetooth on my phone which then causes my phone to not work in handsfree mode (ok google etc.) Furthermore the backup camera does not always work. Finally, a Toyota Service Manager told me in his oppinion 25% of the entune units are "bricked," non functional and need to be replaced. Toyota has fired according to him Panasonic as the supplier and now has a new provider.

The padding in headrest is unglued and coming loose. My son has grabbed the head supports at times but I noticed it detached one day and not sitting in the correct position. I have tried to reattach but would require heavy duty glue and assistance with assembly. Car seat is available for inspection and I will include pictures. The car seat has not been inspected by police, manufacturer of others.

SRS Airbag warning light is in the dashboard. It is confirmed by the Toyota dealer a faulty radio head unit can not register the SRS airbag. In the event of collision, the SRS airbag will not be deployed, causing passengers at risk.

car seat gets loose easily and quickly with simple vibration down the road. example going over speed bumps and appropriate speed causes the seat to oscillate making seat belt strap loose that keeps the car seat in place. in one instance the strap was so loose i could not use the rotate function without the possibility of the car seat falling out of place. this is the second unit i have had with the issue manufacture is not moving to fix issue.

The contact stated that the release button on the crotch restraint buckle, where the two arm and chest belts buckled, were cracked into two pieces rendering the car seat unusable. The failure occurred inside of the vehicle. The child was not in the car seat at the time of the failure. The manufacturer was made aware of the failure. The car seat was not replaced.

The contact had ordered the vehicle with modifications due to being a disabled veteran from a specialty vehicle modification for disabled persons manufacturer. The contact had picked up the van from a dealer and was driving approximately 60 MPH when the smell of something burning filled the passenger cabin. The contact pulled over and called the dealer who took the vehicle back and determined that the fuel line was rubbing against the rear passenger side tire. The vehicle was repaired. The contact stated that also the "check battery" light had illuminated, the light blacks out the entire instrument cluster and cannot be reset. The contact stated that he took the vehicle back to the dealer and they could not repair the warning light failure. The dealer had diagnosed that the main batteries were getting overheated making the vehicle dangerous to drive. The manufacturer had not been informed of the failure. The contact stated that the van modification facility had modified the vehicle with all of the defects and sold the van with the defects to him. The failure mileage was approximately 9,000.

After starting the van, an error pops up: "SRS Airbag System Malfunction Visit Your Dealer." This occurs a day after turning the car on and off and driving for several hours. The next day, it may come back on, or it may not. I have taken it to a dealership, and they stated it was a firmware update they completed. After they updated the warning light, it came back on less than a week later.

I have a Toyota sienna 2022 Limited AWD, not been driving much recently. The last time I drove was 2 days ago but local around 2 miles. Today I tried opening my doors, and there is no response at all. At first I thought maybe my key fob is dead, so I grabbed my spare and tried, nothing. Pull both my physical keys out twist and turned, still nothing. Called Toyota Roadside service and they said in that case they will have to tow my van to the nearest dealer, so it seems Toyota is aware of this issue. What is the point of the physical keys if they can't open your doors when your battery is dead? What if my kids are stuck in the car????

When engaging the cruise control, the vehicle continued to accelerate even after I let off the button. During a trip with this vehicle on September 14, 2024, this vehicle did this twice. I have never driven another vehicle where this occurs. If this is supposed to be a "feature", it's unsafe and should not be legal. If it remains legal, there should at least be some kind of safety sticker.
