We purchased a 2024 Tesla Model Y approximately one month ago. We have experienced multiple occurrences (10-15) when Traffic Adaptive Cruise Control was engaged. Our car suddenly braked for no apparent reason. Last night, while traveling at 65 mph, on a four lane major highway, it happened again, nearly causing a rear end collision. We are hesitant to use cruise control because this feature (T.A.C.C.) is built into cruise, no way to separate it. After researching online, we discovered this is not a new or unusual event and has been reported by hundreds if not thousands of Tesla owners, dating back at least 3-4 years (if not longer). There are no warnings prior to the event. The car suddenly slams on brakes in what seems to be a “panic stop.” We understand a record of these occurrences are recorded and available from Tesla at your request. We have registered a complaint with Tesla as well.

I was driving down the highway with my wife and baby at about 65 mph. I was using regular cruise control with no one in front of me when all of a sudden, the car braked extremely hard. Luckily, the car behind me was far enough back to switch lanes and pass me. If it was closer, this could have resulted in a highway accident. The car stopped braking maybe around 30 or 40 mph. I may have it recorded in the Tesla camera system since I pressed the save button approx 4 min after it happened. I also scheduled a service call with Tesla that will hopefully happen on may 7th

BACK OVER PREVENTION: SENSING SYSTEM: CAMERA

Components Failing - Reported as central computer. This made the vehicle turn signals and backup cameras fail as critical components. These two elements were/are completely non-functional. Additional less critical safety functions that have been disabled include blind spot monitoring and cameras, both forward and reverse emergency braking, and pedestrian alerts. My safety was put at-risk due to camera failures while driving and backing out of parking spaces. These vehicles are designed with poor and minimal visibility due to their cameras compensating for visibility shortcomings. You cannot adequately see proximity to other vehicles or pedestrians when cameras are not active. This particular impacts reversing when backing in and out of parking spots since the rear windshield is so small and poorly angled. Drivers have reported that the cameras for them are failing mid-drive leading them to not know where other vehicles were next to them and making unsafe lane changes thinking their warning systems were active. The vehicle component has NOT been inspected by anyone as there was thankfully no actually accident or incident due to the failure. Tesla has not inspected the vehicle due to NO availability for 3+ weeks and they will not consider human contact to expedite review. There are now 100s of community reports of the same defect on online communities all showing failures on same-day or within 24 hours of each other. There were no warnings prior to the failure. The symptoms simply stopped working. It required a phone call to Tesla support to even receive an accurate error code. Tesla is claiming a "short" in the main computer system of the vehicle. This occurred for myself and dozens of others all within several hours of each other in the early morning hours of Friday November 29th 2024. Suspicion is that that this was due to a software update from Tesla.

All Cameras not working with latest Software update - told need new AP4 computer on 2024 model Y which is 2 months old. Tesla service advisor states - car is totally safe to drive with just mirrors till appointment in few weeks. No backup camera, navigation, battery life dying faster. It seems like a liability to me to drive this way and very negligent to me.

Vehicle warning icons are blocked by the drivers right arm. In some driving positions, the current vehicle speed is also blocked. Recommend shifting the icons toward the center of the monitor.

LANE DEPARTURE: BLIND SPOT DETECTION

This is a complaint regarding a malfunction of the Tesla Full Self-Driving (FSD) feature, which resulted in damage to my vehicle and may potentially pose a significant safety risk to public. On 07/02/2024, my newly purchased Tesla [Model-Y 2024] was parked next to a pillar in a parking garage. After ensuring that the Full Self-Driving sign was on, I activated the FSD feature. However, as soon as the function was engaged, the vehicle instantly took off and made a right turn. This sudden and unexpected action caused the right side of the back passenger door to scrape against the iron guardrail on the pillar and caused damages to the right back passenger door and a portion of right back fender. This action happened so sudden that did not have the chance to react and take manual control. I contacted Tesla to report the issue and seek assistance, but after +4 weeks of contacting and restless following up with Tesla service teams, Tesla stated that the software was functioning properly and denied any responsibility for the malfunction, and they closed my case without offering any resolution or further investigation. Luckily, this incident happened in a confined parking garage. It could have resulted in more severe damage or injury had the circumstances been different. Additionally, Tesla's dismissal of the issue and refusal to accept responsibility is deeply concerning, especially about the reliability and safety of their Full Self-Driving technology. I sincerely request your office to investigate into this incident, and I hope you could advise any appropriate action to examine and address this issue. I attached a few photographs of where my vehicle was parked before the FSD was turned on, and the damages on both my vehicle and the guardrail at the time of the incident. I am more than happy to provide any further information or clarification. Thank you for your attention to this matter. I look forward to your guidance and assistance.

Electrical system had 7 alerts/warning scroll on screen. Said vehicle was going to shut down. Regenerative braking system failed. Pulled car into a parking spot. Vehicle would not go into park so I could not take foot off of brake. Started to shift between park and neutral randomly. Was told by Tesla to reboot car. Did this three times, but nothing changed. Tesla dealership reproduced and confirmed. There were no warnings or messages before the incident.

1st incidence: The car with full self driving mode self-changed to shoulder lane on [XXX] for unknown reason. I had to steer back to my lane to avoid hitting the FWY divider. 2nd incidence: with autosteer/cruise control mode, the car self changed to left lane, again I quickly guided the steering wheel back to my lane to correct the action and it caused a car accident. EXTERIOR LIGHTING

The headlamp design of the 2024 Tesla Model Y creates a shadow (or shadows) that cannot be adjusted away. These shadows could be a contributing factor if an accident were to occur. Tesla Service has stated there is nothing more they can do regarding adjustment. Initially, there were two shadows. The user-friendly headlamp adjustment on the touchscreen allows only up-down adjustment after the system self-calibrates. Tesla Service manually adjusted the physical headlamps but did not resolve the shadows. This adjustment merely relocated the shadows while creating an additional horizontal shadow. I believe the reflector design of the headlamp is the issue. This is my third Tesla, and prior models did not have this issue. It is best to see video examples, but this form does not allow the file type and I suspect YouTube links would be blocked/removed. Please Advise.

Turn signals turn off prematurely. Disengaging the cruise control or entering a turn lane after the turn signal is activated appear to turn off the turn signal before reaching the intersection or turn. This occurs whether I use the half or full press on the turn signal stalk. To be honest I'm not sure the difference between the half and full press, since they appear to function the same. This is dangerous for 2 reasons. 1.) Vehicles waiting at the intersection may believe I have changed my mind on turn and take actions on the belief that I am not going to turn. 2.) It creates a distraction as I need to react and take further action to re-activate the turn signal rather than focusing on traffic and the turn.

While driving on highway at around 60,65 miles speed, I saw a warning that fronk is open. I dont think I pressed open fronk while driving and not sure even its possible while driving. Fronk wasnt open when I started the drive for sure else would have noticed the warning. Warning actually started after driving for 5,6 miles. I was on highway and took me some time to find a safe place to get on the shoulder and when I checked fronk was actually open. Im not sure how much an issue it was or if Tesla fronk can get loose and fly or hit the windscreen and cause visiblity issue or accident involving other vehilces but this was something which was a big concern for a moment while driving at high speed.

My Tesla (dark gray) was t-boned by a 2015 Hyundai (white) while it was passing the intersection on green light. The Hyundai hit my car when it tried to turn right on red light. My car was running at around 25 mph at that moment. About one second before the accident, my car's front collision warning went off, but everything was too late; the Hyundai's left front bumper hit my car. The crash did not cause any serious damage on the white Hyundai; as you can see in the photo, it barely got any dents, except its left front headlight cap was broken. The incident created small dents on my car's right fender panel and front right door. However, even though my car's front right wheel was not directly hit, it fell off during the accident. If my car was running at a high speed, it would have rolled over and caused serious injuries.

Automatic Wipers are unreliable, they turn on when no rain is present and they fail to come on when rain is present. Additionally the manual wiper controls are unsafe to use while driving and are not easy to adjust, resulting in a significant distraction while operating a vehicle in the rain. The vehicle also lacks manual Intermittent wipers, unless left in auto which barely works.

The auto wipers come on multiple times a drive without water being on the windshield. This has been dangerous because the wipers will smear material across the windshield at the wrong times creating a visibility hazard. It almost caused a wreck where it activated smearing the windshield while the sun was directly ahead. Tesla’s response was the auto wipers are in Beta mode and I should turn them off to keep them from randomly coming on. This is a challenge since you have to use the center screen to find the wipers and activate them causing one to look away from the roadway.

While driving to work, using FSD, the autopilot steered me into the path of an overtaking vehicle moving at about 20 mph faster than my vehicle. By quickly taking over steering and speed control, I was able to avoid a collision. A couple miles further down the road, the car could not decide on which exit lane to use, and weaved between two possibilities, probably alarming and angering drivers behind me. Again, I had to take over control. Taking over speed and steering control was necessary two more times in my 25 minute trip, most of it on well-marked multi-lane highways. Of the four incidents, only one would have resulted in a serious high speed collision, had I not taken over very quickly. Having coached robotics competitions, and piloted aircraft with autopilot, it is my opinion that the FSD (SUPERVISED) software is at alpha stage in development (not yet up to beta stage, at which it could be safely released only to select skilled drivers, with suitable disclaimers and warnings) . At delivery, no introduction to the car's controls is provided, so customers are on their own regarding finding out, by trial and error and the occasional potentially lethal collision, how to safely drive the vehicle with FSD engaged. The manufacturer should be required to instruct all customers on the very high level of vigilance required to operate with FSD engaged.

The contact owned a 2024 Tesla Model Y. The contact stated while driving 60 MPH he began to lose control of the steering wheel which resulted in the vehicle crashing into a ravine and flipping over multiples with no warning light illuminated. The contact stated that three wheels detached the vehicle but was unsure if the wheels fell before the accident. The contact was taken to a local hospital for a trauma evaluation in which the contact suffered a concussion and bruises on his body. A police report was filed. The vehicle was towed to the salvage junkyard. The dealer was not contacted, and the vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 50.

I was on [XXX] heading towards Chicago and following the traffic. I started changing the line to pass the vehicle in front of me and suddenly my tesla completely lost control and started drifting to the sides. I released the accelerator right away but it made it even worse. It seems that acceleration continues to happen that’s what made car to drift. Eventually car slowed down and I was able to make it to my line. I don’t understand why line assistance didn’t work to keep the vehicle stable, why after releasing accelerator the vehicle continued to gain the speed. It was sunny day and dry road. At night, on paved and well-painted rural roads, where there is no other sources of light, the pillar cameras incorrectly warn they are blocked or blinded. At the same time the Lane Departure Warning and Lane Keeping Assistance stop functioning without warning. On the image below I let the car drift over the line. As I crossed the line, the display should have highlighted the line in blue and there should have been a flashing warning at the top of the screen and an audible warning. None of those occurred. Since lane keeping assistance was enabled. It should have also steered the car back into the lane. I can regularly reproduce the issue. We don't understand why they are using the pillar cameras for this and not the forward cameras that can take advantage of the headlights. It is unsafe, especially since I am not warned of the system is not available.

BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING

After latest Tesla software upgrade, all cameras and GPS stops working. All automatic functionalities of the car stops working without cameras, and it is really unsafe to drive, for example back up the car since the physical rear view mirror is very small, and car is naturally designed with those cameras to function properly.

Widespread issues acknowledged privately by Tesla service managers, advisors, and engineers. Affects all models built from July 2024 to present with most recent “AP4” computer and hardware, primarily out of Fremont factory. Suddenly, computer is short circuiting. This results in MULTIPLE systems failing given effectively ALL systems are dependent upon this “AP4” computer and hardware, and initially causes GPS and Camera sensors to fail also. This system failure includes: -federally required rear view vision system -adaptive cruise control -automatic emergency braking -blind spot warning -forward collision warning -lane departure warning -lane keeping assistance -parking collision warning -rear cross traffic warning -visibility and wiper systems -all external lighting systems -speed control Internally, Tesla service advisors and managers are being told to advise customers that vehicles can be safely driven until fixed - which is currently diagnosed to be a removal and replacement of this “AP4”, which is heavily supply chain restricted. Customers are being told to wait “weeks” - with estimations in January and February - for this to be fixed. Service Advisors and managers are being told to not acknowledge clear safety concerns given vehicles are no longer compliant with all applicable federal laws and regulations, such as those requiring rear view vision field of view expansion, etc. Further, internally Sales advisors and Managers are being told to continue heavily pushing sales to meet record sales targets to impact stock price. Several private conversations regarding ethical concerns of pushing noncompliant and known defective vehicles for stock value and potential criminality. Service advisors and managers also privately concerned regarding legal liability of impacted and defective vehicles in accident, given Tesla is communicating to continue driving. My own personal vehicle has been impacted, and information above has been gathered privately.

Front motor of 3-week-old Tesla Model Y became disabled while driving on a highway at 65 mph. This meant the car also lost power steering, traction control, and ABS braking. Car was limited to top speed of around 40 mph on a busy highway with very little room to pull over. Car was difficult to steer and braking was greatly reduced. There was no advanced warning of this. Was finally able to pull over, exit and reenter the vehicle, which temporarily corrected the problem. However, when the car was started the next day, the front motor was disabled again, but resolved when the car was restarted. The car was not driven again until Tesla service center was contacted and car was taken in for repair. On the way to the service center, the issue occurred again. The alerts included “Front motor disabled - OK to drive. Vehicle power may be limited” The vehicle is not safe to drive since this is happening while traveling at high speeds. Tesla service has not addressed the issue or tried to diagnose the cause yet in spite of the car being at their service center for over a week.

The contact owns a 2024 Tesla Model Y. The contact stated that while driving approximately 15 MPH the vehicle suddenly experienced unintended acceleration causing the driver to lose control of the steering and crash into a second vehicle. During the crash, the frontend of the vehicle was severely damaged but the air bag did not deploy. The contact sustained a right-hand injury that did not require medical treatment. Three occupants from the second vehicle sustained undisclosed injuries and were transported to the hospital. A police report was taken at the scene and the vehicle was towed away. The cause of the failure was not yet determined. The manufacturer and local dealer were notified of the failure. The failure mileage was 7,608.

LANE DEPARTURE: WARNING

In a 90 mile drive I experience phantom breaking 4 different times. No car was in front of me, in fact nothing at all was in front of me when my car slammed on the brakes 4 separate times. One time there was a car somewhat close behind me. I also twice experienced audible warnings and flashing alarms to take control immediately of the car when my hands were on the wheel and I was already in control of the car, it was alarming and shocking to get these alarms while already in control and driving. I believe this car, as currently configured, can pose a danger to other drivers on the road.

The manufacturer was not notified of the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 3,000.

The incident created small dents on my car's right fender panel and front right door. However, even though my car's front right wheel was not directly hit, it fell off during the accident. If my car was running at a high speed, it would have rolled over and caused serious injuries.

I have had this car since May 2024 and the current mileage on the vehicle is 7,130 miles. In the past few weeks, I have had to completely replace both rear OEM aluminum alloy wheels due to cracks caused by normal road driving, which caused both rear tires to go flat. Per Tesla, this costly replacement is not covered under their warranty. I am now learning that this is a common issue with these particular wheels on this particular model (Model Y Performance). It appears the aluminum alloy wheels are simply not sturdy enough to bear the weight of the vehicle and the required tires simply do not provide sufficient cushioning to prevent wheel cracks resulting from normal wear and tear/road driving. Many customers have had to replace the OEM wheels with aftermarket wheels made of forged steel. These customers report zero issues with the much stronger wheels. This appears to be a product design or manufacturing problem, which is creating a significant consumer/public health risk.

Front motor of 3-week-old Tesla Model Y became disabled while driving on a highway at 65 mph. This meant the car also lost power steering, traction control, and ABS braking. Car was limited to top speed of around 40 mph on a busy highway with very little room to pull over. Car was difficult to steer and braking was greatly reduced. There was no advanced warning of this. Was finally able to pull over, exit and reenter the vehicle, which temporarily corrected the problem. However, when the car was started the next day, the front motor was disabled again, but resolved when the car was restarted. The car was not driven again until Tesla service center was contacted and car was taken in for repair. On the way to the service center, the issue occurred again. The alerts included “Front motor disabled - OK to drive. Vehicle power may be limited” The vehicle is not safe to drive since this is happening while traveling at high speeds. Tesla service has not addressed the issue or tried to diagnose the cause yet in spite of the car being at their service center for over a week.

On Wednesday, [XXX], 2024 between 1015-1045 am while traveling on [XXX] , my vehicle unexpectedly accelerated while entering the left turn lane. On ramp to [XXX] northbound, Monroe county, Pennsylvania. At the time I was giving steering input and had also applied the windshield wiper system. This happened a second time while approaching the intersection of [XXX] northbound. While taking my foot off the accelerator pedal to allow for regenerative braking and using the windshield wipers the vehicle accelerated at a quick rate. In both instances I had to apply the brake to slow it down. I noticed Tesla on that date. Tesla related that someone would contact me. No co text has been made as of the submission of this report. Whenever turning a clicking sound is occurring underneath the driver seat. The seat does not seem to be properly secured from the factory. Tesla said it does not seem to be a safety concern over the phone and will not work to get my vehicle into the service department in a reasonable timeframe. When removed, the hot, sticky parts of the seat stuck to my bag and hands. I was put at risk for potential burns. oA person could get stuck on the seat potentially putting them at risk when not being able to exit the vehicle if needed in a safety situation. oThe heat controls could be shorted in the seat potentially putting anyone near at risk of a heat or possible fire incident. •Has the problem been reproduced or confirmed by a dealer or independent service center? oNot that I am aware of. BACK OVER PREVENTION: WARNINGS

This is a complaint regarding a malfunction of the Tesla Full Self-Driving (FSD) feature, which resulted in damage to my vehicle and may potentially pose a significant safety risk to public. On 07/02/2024, my newly purchased Tesla [Model-Y 2024] was parked next to a pillar in a parking garage. After ensuring that the Full Self-Driving sign was on, I activated the FSD feature. However, as soon as the function was engaged, the vehicle instantly took off and made a right turn. This sudden and unexpected action caused the right side of the back passenger door to scrape against the iron guardrail on the pillar and caused damages to the right back passenger door and a portion of right back fender. This action happened so sudden that did not have the chance to react and take manual control. I contacted Tesla to report the issue and seek assistance, but after +4 weeks of contacting and restless following up with Tesla service teams, Tesla stated that the software was functioning properly and denied any responsibility for the malfunction, and they closed my case without offering any resolution or further investigation. Luckily, this incident happened in a confined parking garage. It could have resulted in more severe damage or injury had the circumstances been different. Additionally, Tesla's dismissal of the issue and refusal to accept responsibility is deeply concerning, especially about the reliability and safety of their Full Self-Driving technology. I sincerely request your office to investigate into this incident, and I hope you could advise any appropriate action to examine and address this issue. I attached a few photographs of where my vehicle was parked before the FSD was turned on, and the damages on both my vehicle and the guardrail at the time of the incident. I am more than happy to provide any further information or clarification. Thank you for your attention to this matter. I look forward to your guidance and assistance.

All safety sensors appear to be out of service. Navigation is stuck at one location and is not working. All cameras, including backup, are out of operation (only shows dark screen). Back up sensor does not work. This issue has been ongoing for 1 month on a brand new vehicle. Tesla service center has no ETA on resolution. Service appointment has already been pushed back to next year (Jan 2025).

The battery coolant started leaking when we connected the car to a supercharger. After charging a low coolant warning came up on the screen, but the message read “OK to drive”. Started driving home when more alerts came up “Power reduced” and “Speed limited to 68 mph” but still showed “OK to drive”. About 30 miles later the screen abruptly shut down on the freeway. Was able to pull over to a stop on the shoulder and call a tow truck. Screen never came back up even after attempting to restart, car is now at the service center. There is a pump that has continued running for over 2 days without stopping. The interior lights turn on and off every 30 seconds. There has been no damage from road debris or anything else to this car. The vehicle only had ~350 miles on it, and we only had it for 3 days before it malfunctioned. The screen displays vital information such as speed, turn indicators, and so on. Without the screen being on it is not possible to safely operate the vehicle.

While driving on highway at around 60,65 miles speed, I saw a warning that fronk is open. I dont think I pressed open fronk while driving and not sure even its possible while driving. Fronk wasnt open when I started the drive for sure else would have noticed the warning. Warning actually started after driving for 5,6 miles. I was on highway and took me some time to find a safe place to get on the shoulder and when I checked fronk was actually open. Im not sure how much an issue it was or if Tesla fronk can get loose and fly or hit the windscreen and cause visiblity issue or accident involving other vehilces but this was something which was a big concern for a moment while driving at high speed.

My Tesla (dark gray) was t-boned by a 2015 Hyundai (white) while it was passing the intersection on green light. The Hyundai hit my car when it tried to turn right on red light. My car was running at around 25 mph at that moment. About one second before the accident, my car's front collision warning went off, but everything was too late; the Hyundai's left front bumper hit my car. The crash did not cause any serious damage on the white Hyundai; as you can see in the photo, it barely got any dents, except its left front headlight cap was broken. The incident created small dents on my car's right fender panel and front right door. However, even though my car's front right wheel was not directly hit, it fell off during the accident. If my car was running at a high speed, it would have rolled over and caused serious injuries.

The contact owns a 2024 Tesla Model Y equipped with Continental Tires, Tire Line: Pro Contact, Tire Size: 255/45/R20, DOT Number: (N/A). The contact stated that several weeks after purchasing the vehicle, a neighbor informed the contact that a tire was flat. The contact attempted to pump air into the tire; however, the air immediately seeped out. Due to the failure, the vehicle was towed to an independent mechanic where the tire was replaced with the same brand tire. The contact stated that almost a month later, the tire pressure indicator light appeared on the infotainment screen while the contact's wife was driving the vehicle. The contact was called to the scene and attempted to inflate the rear passenger tire; however, the air seeped out of the tire. Upon inspection, the contact discovered multiple cracks along the inner sidewall which caused the tire to peel. The vehicle was towed back to the independent mechanic where the vehicle remained. The vehicle manufacturer was notified of the failure and referred the contact to the tire manufacturer. The tire manufacturer was notified and informed the contact that a tire representative would need to inspect the tire. The vehicle was not repaired. The tire and vehicle failure mileage was approximately 550.

With beta self driving engaged, I turned south on [XXX] intending to turn west on [XXX]. At [XXX], there are 2 right turn lanes. I approached the intersection turning from the left lane. Rather than turn right, the car continued straight. I was able to manually override the system and luckily brought the car under control; otherwise, I am sure I would have ended up in a very bad spot. Later the same day beta did not recognize a turn lane into my community but rather proceeded straight. I have since disabled the beta FSD.
