Driver assist forward continues to have issues after being at dealer for issue for 4 weeks for repair After getting car back from dealer for the first three days the error did not come up. Now the error comes and goes daily. Issue happens when approaching stop lights stop signs where there is nothing in front of you starts vibrating seat and slams on breaks. Same stop sign as yesterday but today it vibrates warnings Makes not sense.

Multiple components of our driver assistance software have failed, beyond what we can check in the fields above. Most recently, we have experienced two major failures. The first failure occurred when the car triggered the emergency parking brake on its own while we were driving in traffic at 30 mph. We narrowly avoided being rear ended. This is the incident I've detailed in the fields below. The second failure occurred when the adaptive cruise control failed to brake as we approached gridlock traffic. We again narrowly avoided collision on multiple fronts. We were going approximately 45 mph with roughly 2000 miles on the car. We have the service records, but the selling dealer and Cadillac engineering claim these issues are now fixed. How was your safety or the safety of others put at risk? We were well within less than a car length of collision in both of the situations described above. This car is a large SUV, it would presumably cause significant damage to cars it hit. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. They verified failures and out-of-date software in our vehicle in this last service visit. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? This vehicle has been directly inspected by Cadillac engineering through our selling dealer. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The warning lights we experienced included the warnings that the system had engaged and "service driver assist system." There was no warning when the adaptive cruise control failed.

Without warning and while driving this vehicle on expressway, I've received the following notifications several times: "Brake Failure" (car brakes on its own); "Service Stability and Auto Hold"; and "High Voltage" and Battery. Brakes, speed, steering, parking systems and camera do not work properly. Car's capacity to drive vehicle safely is reduced significantly - speed to drive car is immediately reduced and is limited without warning. Car shakes passengers violently (whiplash) when it believes there are people or objects in its path when there is nothing there and does not work properly when pedestrians are around the vehicle. During the incidents both my safety and other drivers safety are at risk - I am afraid to drive this vehicle although the dealership has indicated they have corrected the errors.Vehicle purchased 23 Oct - problems began around 2300 miles and have continued thru 4200 miles.

EXTERIOR LIGHTING

Headlights on vehicle “droop” after driving 20 minutes or more. They start off okay and then become non existent l, pointing directly down toward the pavement in front of the vehicle and you can’t see more than 1 car length in front of you. You must turn the vehicle off and then back on to reset the headlights to drive. This is a huge safety issue for driving at night.

Headlights start dropping as car is driven. Every 10 to 15 miles must pull car over and restart as you can not see further than a car’s length in front of you. Cycle car off and then back on the headlights return to normal for another 10 miles. Also seems that some of the individual lamps start pointing upward blinding oncoming drives even with most of the lamps are pointing down in front of the less than a cars length. Seems the headlights are making adjustments be are not correct until you do a power cycle.

The automatic collision system braking is engaging when the vehicle is in reverse and while it is raining. This occurs multiple times while trying to back the vehicle up during rain. This is dangerous for the passengers since they are abruptly thrown backwards without warning. The vehicle must be placed in "park" and then "reverse" multiple times until the collision braking stops malfunctioning.

It happened two times to me I was driving and I lost acceleration power. I put the car on Park and back to drive and it still didn't help the only way to get it to drive is to restart the car

While driving the 12V battery system failed as well as the high voltage charging system. The vehicle lost traction control in the rain and was slipping on the road. The forward collision detection also intermittently goes out. If you are in one pedal driving mode and using adaptive cruise control when the forward collision detection goes out it will deactivate adaptive cruise control and apply enough regenerative breaking to rapidly slow the car. If the driver does not have their foot on the accelerator when this happens and you are in traffic it could cause an accident. The vehicle is not saving any diagnostic information regarding these failures and dealerships will not work the failures without diagnostic codes.

While driving northbound on [XXX], south of Oklahoma City, at normal highway speed, at night, in the rain (on [XXX]), this EV lost all power to the wheels and the regenerative braking ceased to function. It appears that all flow of electrons from the batteries to the drive motors, and from the regenerative brakes to the batteries stopped. I still had dashboard lights, steering, and conventional brakes. I was able to safely coast to a stop on the highway shoulder. I set the parking brakes, put the “transmission” into “park”, and turned off the power button. I then restarted the car, and all was normal again. I continued my drive from Austin, TX, to Evergreen, CO with no additional incidents, arriving on [XXX]. This is reminiscent of the “blue screen of death” in old Microsoft desktop computers — fixed the problem by cycling the ON / OFF switch. Unfortunately, doing this when driving at 75 mph is less than ideal, and is a real potential safety problem. On or about 11/14/24 Car found completely dead and won't start in garage. Car had about 50% charge in main battery pack and 12V completely dead. Called OnStar service and they had vehicle taken to dealership on flatbed. GM app reports Vehicle Issues: "Electric Drive Unit, Lithium Ion Battery, Electric Drive Unit, and 12V battery require your attention now." Vehicle purchased on or about 8/18/24 and at time of failure car had about 385 miles. Car kept garaged when not in use in pristine condition. Dealer reports as of 11/23/24 they do not know the problem with the car. I have been without use of my only car since 11/14/24.

Driving my car on the highway at speeds of about 65, and the charging port door suddenly opens. When I slow down then the door closes on it own. Seems like the housing for the door isnt made tight enough to stay closed during highway driving.

Continuing problem with both front and rear emergency braking. Primarily when it is raining and vehicle is put in reverse violent and unexpected emergency braking will occur for no apparent reason. This will happen several times. Also, when backing out of driveway on a slant, rear emergency braking seems to sense the street and brakes are applied violently. This situation has also happened going into driveway on a slant for front emergency braking. Have informed the dealer several times of this problem and they say that Cadillac has not fix. Very distressing and dangerous for whip lash of adults and children.

The contact owns a 2024 Cadillac Lyriq. The contact stated while driving at approximately 3 MPH as she was entering her garage to park the vehicle, she depressed the brake pedal, however, the vehicle continued forward and crashed into a tool chest located at the back of the garage wall. The contact stated that the front of the grill and the front bumper were damaged and had to be replaced. The contact stated no warning light was illuminated. The contact stated that failure reoccurred when she was parking the vehicle in a parking lot. The contact had taken the vehicle to a local dealer, where it was diagnosed and determined that the ABS system had failed and caused a loss of braking ability. The contact was advised by the dealer that the failure was related to NHTSA Campaign Number: 24V589000 (Service Brakes, Hydraulic). The contact had the recall completed at that time. The manufacturer was informed of the failure. The failure mileage was 3,000.

I purchased a brand new Cadillac Lyriq on July 1, 2024, and have encountered several severe issues that compromise the safety and reliability of the vehicle: 1.Display Failure: The display frequently flickers, flashes, and sometimes completely blacks out, causing the loss of critical information such as speed, backup camera, navigation, and other vital systems. This makes the vehicle unsafe to drive, as it is impossible to monitor speed and other essential functions necessary for safe operation. 2.Battery Overheating During DC Fast Charging: The vehicle’s battery consistently overheats during DC fast charging sessions, causing the charge rate to drop significantly (from ~190 kW down to 14-20 kW). This overheating issue not only drastically extends charging times but also raises serious safety concerns, particularly during long road trips in hot weather. The vehicle has repeatedly experienced these issues, making it unreliable for long-distance travel. 3.Charger Overheating and Instability During Home AC Charging: The home AC charger provided with the vehicle becomes excessively hot while charging the Lyriq. Additionally, the charging rate fluctuates erratically between 0-3 kW, further complicating the charging process. This overheating of the charger at home raises concerns about the safety of parking or charging the vehicle in my garage, as it may pose a fire risk. These issues have been reported to my dealership, who have attempted repairs without success. The problems persist, and the vehicle remains unsafe and unreliable for my family’s needs. I am deeply concerned about the potential for these issues to lead to more severe safety incidents if not addressed promptly.

This is an update to my original NHTSA complaint submitted on May 5, 2025 (complaint number 11658852), regarding a complete electrical failure in a 2024 Cadillac Lyriq. While parked in a garage, the vehicle experienced a full shutdown—rendering all systems inoperable, including the door locks and windows. Most alarmingly, a [XXX] child was trapped in the back seat due to the doors being electronically locked and unresponsive. The dealership has since diagnosed the failure as a cable routed improperly near the steering column that had rubbed against a component and snapped. This severed connection resulted in the complete loss of vehicle power and emergency functions. Since that time, I have discovered multiple similar reports from other Lyriq owners online describing full system shutdowns, locked doors, and failures to start—many of which are shared on public forums such as Reddit [XXX] ). This suggests that this may not be an isolated case but rather a potential systemic defect in the Lyriq's wiring harness design or electrical architecture. I am extremely concerned that this failure mode could occur while driving, particularly at highway speeds, and would result in loss of steering, braking assist, and emergency exit options. I urge the NHTSA to investigate this issue further to determine whether a recall or manufacturer communication is appropriate. This vehicle is currently with the dealership, and I have refused delivery until GM provides formal safety assurances. While driving through Wisconsin on [XXX] with Super Cruise activated in 70MPH traffic the car provided me with driver's side collision warning failure notice and then a forward collision warning failure notice, but the car continued to be operating normally. A short time later a brakes overheating warning came on while the car still seemed to be operating normally and maintaining speed. I immediately disengaged the cruise control to check out the situation and the car instantly locked on the brakes, and we could see smoke and smell the burning brakes. I pulled to car to the side of the road, and we exited the car concerned there might be a fire coming from one of the 4 smoking wheels. The rotors were red hot. We remained a safe distance from the car until the brakes stopped smoking. After it appeared safe, I reentered the car and determined the brakes (parking brakes) were fully engaged and the car would not move. We contacted OnStar for assistance and waited. After a significant time (it seemed like quite a while) I suddenly heard the sound of the parking brakes electrically releasing. Shortly after that I started and tested the car again and it would move so we slowly drove it off the busy interstate to the next exit a short distance away and continued working with OnStar to obtain a tow to a Cadillac dealership. After working with OnStar for several hours and determining that there would most likely be no tow truck coming, we decided to test drive the car around the area now that the brakes had cooled off. We ended up renting a 'safety' car that my wife would drive, and I would try to drive the car back to Minnesota to our dealer. The car operated normally for the drive home, but we did not use any cruise control just in case. It was my belief that a system communication issue told the brakes to engage and that since the Super Cruise was engage the motors just added more power to over-come the drag on the brakes until they over heated. - Software failure resulted in loss of steering control, speed control, and brakes became unavailable. I was on highway car was at 65 mph and then I got a bunch of error messages mentioned below- 1) Steering assist not available 2) AWD is disabled 3) Headlamps not available 4)Service brake assist And then I was not able to brake the car, and steering became highly unresponsive. I took my foot of gas and let the car roll for 1 mile before stopping safely on shoulder, from where I called Cadillac for support (they towed the car back), since error didn't resolve after stopping or rebooting the car. I took screenshot of these errors after stopping and attached them in this email. - Yes it put safety of me, my passenger, and other vehicles at risk, loss speed and steering control you have cause me to hit any other car. - Yes problem is reported to manufacturer, car is in service. - Vehicle is with Service, since no one got hurt, we didn't involve police. - No prior message or warnings we recieved, all errors come as soon as the issues appear.

The contact owns a 2023 Cadillac Lyriq. The contact stated that while reversing and proceeding to make a U-turn, he crashed into a steel guardrail. The automatic braking system did not engage. There was no audible or visual alert or warning provided by the system. There was no injury, police report, airbag deployment, or fire. The vehicle was taken to the local dealer. After twelve days, the vehicle was not diagnosed or repaired. The manufacturer was contacted, who confirmed the vehicle had functioned as designed. The failure mileage was 8,600.

on Sept 23, 2025, I was rear ended by a driver going between 50-60 mph. I am concerned that some safety features were potentially not working at the time of the incident. I will detail the information below: Background / Original Service Issue: When I initially brought my vehicle to Titus-Will Chevrolet GMC Cadillac for service, I observed that multiple critical safety systems were nonfunctional, including, but not limited to: Forward collision alert sensor All cameras (rear, front, top down) Blind zone steering assist Lane departure warning and other associated warning sensors The dealership kept my car for two days and then informed me that a "module" needed to be ordered for repair, but stated that the vehicle was considered “drivable” and would call me when the part came in. I noticed some of the safety features had resumed working, however some were still non-functional. At no point was I told there were any potential safety risks, nor which safety systems were still not functional. I was not given any paperwork, as they were holding it until the module came in. Crash Event: On 09/23/2025 at 5:37 pm, I was involved in a crash. At the time of impact: The forward collision alert sensor did not function I received no warning of the impending collision No airbags deployed. This may have been directly related to the previously identified and unrepaired safety system issues.

I am writing to report an issue with the windshield durability on my 2024 Cadillac LYRIQ. Over the past three weeks, I’ve experienced three separate instances where small rocks struck the windshield while I was driving, each impact resulting in a crack or chip. In my experience with previous vehicles, similar impacts would rarely, if ever, result in such immediate damage, which raises concerns about the material or design of this particular windshield. This frequent damage has quickly become costly and frustrating. Windshields should withstand small impacts to a reasonable extent, and this does not seem to be the case here. I am also reaching out to Cadillac to address my concerns, but I wanted to report this issue to NHTSA in case other drivers are experiencing similar problems. Please let me know if there is further information I can provide. Thank you for looking into this matter.

The phone charging cradle, Bluetooth, wifi, and Radio are getting disconnected, repair made 3-4 times, and still the same problem, also dashboard screen getting dark or disconnected while driving the car. On 01/09 2025, 10 pm I connected to charge the battery and it didn't charge the whole night when I started the car, and heard a big bang like a bomb blew the electrical battery in my garage, thank god I didn't get injured or damage to my garage/home. I took the 5th time to the dealer and asked General Moror to buy back the car. I have filed a written complaint with BBB OF NEW JERSEY and Lemon Law. Thank god I wasn't driving the car on highway when the car battery blew up otherwise I would have or other people would have gotten injured or dead.

Front doors have significant “bounce back” when you open them. You have to block the door from closing/slamming on your leg. The door has hit myself, my passengers and my grandchildren in the leg/shin.
